SCHEDULING AND PAYMENT
Payment for consultation services, as well as any associated laboratory fees, is required at the time of service. Accepted forms of payment include cash, debit cards, credit cards, Apple Pay, and Google Pay.
Please note that CML Health does not participate in sliding scale arrangements or federal assistance programs, and is not affiliated with any government agencies.
CHANGES OF APPOINTMENTS DATA
To make any changes to your personal demographics, emergency contact, or authorized contact information at CML Health, you are required to do so in writing and in person at one of their locations. This policy ensures that your information is securely and properly updated on file. It is best to bring appropriate identification and supporting documentation when visiting the location to request changes.
In case of cancellation, the buyer can proceed to a name change of their appointment, provided that an email has been previously sent, specifying the buyer’s full name and the date of the new appointment.
The buyer must also send other personal data, such as a telephone number. It should be noted that Centro Médico Latino does not issue refunds for cancellation of appointments, nor is it responsible for changing the date when an appointment is not available.
CANCELLATION
In the event that a patient is unable to keep an appointment, it is the patient's responsibility to notify CML Health 12 hours in advance of the scheduled appointment in order to cancel it. This allows the clinic to schedule another patient in that time slot.
Patients with (3) or more appointments not cancelled 12 hours in advance within a 12 -month period will be reviewed by management and may be dropped from the practice due to non-compliance with this policy.
MEDICAL APPOINTMENT: Patients who do not show for their appointment and do not cancel 12 hours in advance will be charged a $25.00 fee, which will be added to their account.
ULTRASOUND APPOINTMENT: Patients who do not show up for their appointment and do not cancel 12 hours in advance will be charged a $50.00 fee that will be added to their account.
Per our clinic's policy and respect for our patients, patients are asked to arrive 15 minutes prior to the scheduled time, otherwise your appointment will be rescheduled to a later time (based on availability) or to the first available day on the calendar.
It is established in our Private Policies described that the patient is responsible for the payment and understands that CML Health does not generate refunds for any reason for the appointment scheduling service.
